Within 4 weeks of the Assessment being carried out an initial rehabilitation report will be delivered. This includes a full statement on functional and neurological needs in addition to the potential and predicted clinical outcome of rehabilitation. The report includes the proposed home based rehabilitation programme, the timescales and the costs.
Rehabilitation for brain injured patients is a financially prudent investment. The costs are agreed in advance and the 8 weekly interim reports contain an up to date picture of the benefits of the rehabilitation programme as well as the expenditure incurred.
Once a programme has commenced a formal interim progress report is provided to the purchaser every 8 weeks. We believe that regular communication is vital to the success of the programme so more frequent updates can be provided to the purchaser if required.
Formal reports are supplemented by informal contact with all stakeholders, regular clinical supervision provided for Reach staff and regular meetings between the client and the Programme Manager.
